Best Eye Drops for Dry Eyes: A Complete Buyer and Care Guide

The thing five buying guides forgot

Five pages rank highly for this phrase and contain about nine thousand words in total. None of them cites any clinical trials, and none has more than a suggestion that the recommendations on it have been tested. Even the page written for the American Academy of Ophthalmology, which is unquestionably the authority on the subject, asks you to take its word for it.

This guide will not attempt to rank eye drops by brand. There are two reasons for this. First, a ranking is only as useful as the moment it is printed, and we would have no good way of updating it as new formulations arrive or old ones get discontinued. Second, we make an eye drop, and you are reading this because you chose to read a page from our website.

A categorisation, however, is possible. Here is a version of the question it answers: how does an eye drop compare to the natural tear film?

Three layers, and the whole map is inside them

The tear film is not water; it is a hydrogel about the consistency of a soap bubble that gets re-formed anew with every blink. It has three layers.

The outermost layer consists of oil produced by meibomian glands, of which there are about thirty per eyelid, arranged vertically like tiny pendants along the lash line. This oily material coats the lacrimal water to prevent it from evaporating as quickly.

The lacrimal water or aqueous component originates in the lacrimal gland, which sits in a depression at the lateral end of each eye. That is where most people would expect tears to come from, and it does produce the liquid portion of the tear film. However, the aqueous layer has salts, proteins, and oxygen and is the source of moisture for the cornea, which has no vascular supply of its own.

The innermost layer, which covers everything and sticks to the cornea, is mucin. It is produced by goblet cells in the conjunctiva. Its property is liquidity, which allows the tear film to spread over the eye surface evenly. Without it, tears bead up and roll down like rain on a waxed windscreen.

Two ways a tear film fails

Three layers, but the film fails in two directions. If the oil layer is thin or the meibomian glands are obstructed, the water underneath evaporates too fast, and that is evaporative dryness. If the lacrimal gland simply produces too little aqueous fluid, the film is thin from the start, and that is aqueous-deficient dryness.

It is not unusual for patients to exhibit symptoms of both evaporative and aqueous deficiency simultaneously. Evaporative dryness is the pattern associated with meibomian gland dysfunction, often associated with prolonged screen time, reduced blink rates in the presence of air conditioning, and dusty commutes.

Autoimmune conditions such as Sjogren's syndrome, as well as various medications with anti-cholinergic properties, are associated with reduced aqueous production. The connection between drugs and aqueous-deficient dryness is particularly relevant, as medications such as antihistamines or anti-depressants are often taken for months with little awareness that they may interfere with tear production.

A dry eye diagnosis is always a personal one. You should consult an eye doctor to determine if and how your tear film is dysfunctional. This guide can help you understand the ingredients in lubricating eye drops and how they interact with the tear film.

What is actually dissolved in the bottle

A few words of caution before proceeding: most over-the-counter eye drops are solutions of saline and either cellulose polymers or hyaluronic acid with some additional lubricants. The list of ingredients is usually found on the packaging, often in small print on the back of the box.

Cellulose polymers: CMC and hypromellose

Carboxymethylcellulose or CMC is the most common thickening agent in Indian eye drops; it may also be listed as carmellose sodium or sodium carboxymethylcellulose, depending on the manufacturer. Hypromellose or HPMC is a related cellulose derivative available as an eye drop. Both thicken water into a viscous solution, allowing it to spread over the eye and keep it moist for a prolonged period.

CMC eye drops are available in several viscosities, and higher viscosities are noticeably thicker. Higher viscosity implies more blurring after instillation and a longer duration of action, which will need to be taken into account if the drop is used for close-range visual work.

Sodium hyaluronate

Sodium hyaluronate is another popular thickener. Hyaluronic acid has an extraordinary affinity for water, and the polymer becomes significantly thinner when exposed to mechanical stress, such as blinking. This property allows the drop to retain its consistency for an appropriate amount of time without being uncomfortable or hazy after a blink.

Glycols: PEG 400 and propylene glycol

Polyethylene glycol 400 and propylene glycol, also known as PEG 400/PG, turn up together in a lot of formulations, often alongside a cellulose polymer. They are humectants, which means they hold water at the surface rather than adding much thickness of their own.

Lipid-containing drops

The milky-looking lubricants contain an emulsified oil either in the form of mineral oil or phospholipids. Such eye drops supplement the outer layer of the film rather than the middle one.

The reasoning is straightforward. If the lid over the water is the part that is missing, adding more water addresses the feeling for a few minutes and leaves the mechanism exactly where it was. Lipid drops exist for the evaporative pattern specifically, and they blur vision for a moment on instillation, because an emulsion is not optically clear.

Gels and night ointments

Gels contain a greater amount of carbomers, polymers that increase in volume when placed in water, and form a soft, stretchy film over the eye. Ointments are the thickest formulation available; they contain petrolatum and mineral oil and stay put for hours.

Both gels and ointments blur vision severely; ointments, in particular, should be reserved for overnight use or for patients who are unable to fully close their eyelids during sleep. Taking an ointment before bedtime at eleven is an obvious choice, but using it before riding a motorcycle in the morning is not advisable.

The finding the other five guides skipped

This guide has discussed the nuances of tear film physiology, the mechanisms by which dryness develops, and the available treatments. A review published in the Cochrane Database of Systematic Reviews in 2016 analysed forty-three randomised or quasi-randomised trials with 3,497 participants with dry eye syndrome and concluded that "in general, there was uncertainty whether different OTC artificial tears provide similar relief of signs and symptoms when compared with each other or placebo" (PubMed).

Only one comparison came out consistently, across two trials with 175 participants: polyacrylic acid 0.2% outperformed polyvinyl alcohol 1.4% on symptoms. Apart from that, the Cochrane researchers rated the overall quality of the evidence as low, blamed high risks of bias in the included trials, and reported that variation in diagnostic criteria, interventions and measurements left them too little to pool.

They also identified 18 completed but unpublished trials with 2,079 participants, which they describe as a high risk of reporting bias, and they state that further high-quality studies are needed before robust conclusions about individual formulations become possible.

Preservatives: a rationale, not a proof

Multi-dose preparations require a preservative to prevent bacterial contamination after repeated opening and dosing. Benzalkonium chloride (BAK), labeled as such on the vial, is the conventional wisdom, but newer regimens utilize either polyquaternium compounds or oxidative preservatives that essentially fall apart into salt and water when they meet the tear film.

The prevailing advice is that if you're getting more than four or five drops per day, you should opt for either single-use vials with preservatives or a multi-dose preparation with some sort of physical valve that prevents bacterial trackup with each dose rather than a chemical preservative. This is good advice, grounded as it is on a basic mechanistic argument that a surfactant-based preservative used on an inflamed epithelium several times daily for months at a stretch isn't likely to be entirely innocuous.

A 2019 systematic review in Arquivos Brasileiros de Oftalmologia screened 2,028 records, retrieved 29 full papers and found only four studies that directly compared preservative-free with preserved artificial tears, each one small, ranging from 15 to 76 participants (PubMed). Pooling symptom scores, Schirmer results, tear break-up time, burning, foreign body sensation and photophobia, it found no statistically significant difference between the two groups and no side effects attributable to either. The authors put it plainly: evidence proving that preservative-free artificial tears are more effective than preserved ones is lacking.

Their own limitations are in large part responsible for why this advice is as common as it is, but four small studies that don't address the question of long-term outcomes are weak evidence for an absence of outcomes, and chronic high-dose treatment with any of these drops is precisely the regimen in which preservatives are hypothesized to have an effect.

So which category should you buy

The most critical consideration for choosing an eye drop is the ocular surface condition, and the mechanism by which the tear film breaks down should be taken into account. Over-the-counter products aimed at evaporative dryness are the clearest example of that. The majority of commercial eye drops are solutions, typically containing either carboxymethylcellulose or sodium hyaluronate.

Lubricants with an oily component are the one category with a mechanism-specific case behind them for evaporative dryness. They supplement the outer layer of the film rather than the middle one. A solution with an emulsified oil will have a cloudy appearance and will temporarily blur vision, which can be inconvenient if work requires close attention to the details.

Screens, blinking and the 20-20-20 rule

Staring at a computer screen reduces the frequency of blinking, which disrupts the tear film's stability. The film sits longer between resurfacings and has more time to break apart, which is why the fourth video call of the afternoon feels different from the first.

A recommendation that appears in every guide for computer users is to follow the 20-20-20 rule, which suggests that every twenty minutes, one should look at something twenty feet away for twenty seconds. It is easy to assume that a scientific basis supports this advice, but in reality, the 20-20-20 rule was only tested once.

A study published in Contact Lens & Anterior Eye in 2023 asked twenty-nine computer users with dry eyes to use webcams that prompted them to take breaks every twenty minutes for a period of two weeks (PubMed). Behaviour did change, and self-reported symptom scores came down.

However, objective measures did not reveal any improvement, and the benefits were no longer observed a week after the study ended. The design of the study itself is questionable, as no control group was used, so feeling better cannot be separated from expecting to. Take the breaks anyway. Just do not file them under treatment.

Heat and dust make everything worse, which is why the complaint tends to peak around April and May in a way that it doesn't really do in December. Triphala water, ghee, and what the trials do and do not show

Ayurveda has its own set of recommendations for addressing dry eyes, most of which are similar to the home remedies mentioned elsewhere. Infusions of triphala powder are a familiar eye rinse, and gulab jal, a rose water, is a common eye toner. It is also customary to apply a warm compress to the closed eyelids at the end of the day.

Home-strained eye washes are not prepared under sterile conditions and can cause irritation. There are two clinical trials registered in India that examine the effects of ghee-based eye drops.

Both are single-centre trials with no control group and relatively short follow-up periods, so neither can say whether the preparation was the reason anyone improved. What both can report is that the participants in them felt better.

That is the entire claim, and we are not about to invent a larger one inside an article whose whole argument is that this category is sold harder than the evidence supports.

When to stop managing this yourself

Dryness is an annoyance, a roughness and a sting behind the eyes, the occasional watering that comes and goes as it pleases, the vague unhappiness that comes with staring at a screen all day that dissipates when you look away. It's unpleasant, but easily managed.

Any of the following are not dryness.

Pain, rather than discomfort. Photophobia or halos, rather than slight irritation. Discharge or crusting of the eyelids first thing in the morning, especially if it has a yellowish or greenish hue. Redness that persists or worsens day after day rather than coming and going. Any change in visual acuity, blurry or otherwise, that doesn't resolve itself within a few seconds after blinking.

None of these things are things that come with the box, ours or anyone else's, and any of these things happening are a sign that you need to bring up with an ophthalmologist rather than experiment with a new bottle of drops.

Add to that list the worsening of any of the above after a few weeks of consistent use of your preferred drop. None of these things are an indication to simply try a different eyedrop, and all of these things are a sign to make an appointment to look into it more seriously.

If you're a contact lens wearer, or if you're post-surgery and post convalescence, or if you're on long-term glaucoma medication in any capacity, it's a good idea to bring this up with your prescribing physician before you try anything, because these regimens tend to have specific restrictions for any of those conditions.

Look at the back of the eye drop you already have and see what the active ingredient is.

If it contains an extract of cellulose and you are comfortable with it, you should continue using it. Cellulose derivatives are the most common thickening agents among over-the-counter eye drops, and the pooled analysis of clinical trials cited earlier provides little reason to change. If the drop is uncomfortable or used more than four times a day, consider switching to sodium hyaluronate.

If dryness tends to occur after working on a computer or in dry air, opt for eye drops formulated for evaporative dryness.

Add a gel or an ointment to your evening routine for extended relief. Finally, treat any opinion on the benefits and drawbacks of competing eye drops with scepticism.
